Birth Month Reviews - Long Term Condition Monitoring (Chronic Disease Management)
Regular clinics to monitor certain chronic diseases such as diabetes, high blood pressure (hypertension) asthma and COPD are run by our clinical team. It is important to have regular reviews to ensure good control of these conditions to prevent complications developing. Any medications prescribed must be reviewed at least once a year. Patients are invited to book a review in the month of their birth every year -as a birthday treat! If you feel unwell in-between reviews, contact us to book a consultation with a clinician- you don't have to wait until your birth month, we will want to see you again in your birth month too. If you have more than one condition that requires reviews you can get them all done in one visit. Contact us to book your review appointment in the month of your birth, you don't need to wait for an invitation.
Diabetes
Sister Willicombe and sister Laura run Diabetic clinics which are overseen by the doctors. Diabetic patients are normally required to have a blood test a week before having their review appointment. At your review the nurse can offer advice and make a plan with you for your diabetic management. They will discuss your test results, making any changes if necessary. Please contact us in the month of your birth to book an appointment, you don't need to wait for an invitation.
Blood Tests
If one of our clinicians requires you to have a blood test or you are due an annual test for a birth month review, you can book with in with our phlebotomist (blood taker) at Hodnet. Alternatively, we can print a blood form with the details of what is needed for you to take to other locations within Shrewsbury and Telford Hospital NHS Trust (SaTH). Private Medicals and Reports
Certain medicals, reports and forms are not covered by the NHS. These will be charged at the BMA recommended rate. Please ask at reception or telephone the surgery for details.
